express dictate voice record

I am dictating a report (approx. 6 mins long) and then sending it through to my personal assistant to type up through Express Scribe. As she is typing she is noticing that sentences stop mid sentence and then start again in another paragraph (i.e part sentences are missing). We have tried replacing the microphone, deleting the whole program and reinstalling it again, but the problem is still happening.
