My concern is that viewers won't be able to read the text on the forms.  Perhaps I just have a playback problem on my computer?  Are the forms readable to everyone else?

I've tried multiple times to capture my screen for upload to YouTube.  It always works, but the quality is crap.  Can someone give me the recommended settings or tell me what I am doing wrong?  This is my latest attempt to screen capture and upload.
